Called Silver Cab (+62 778 461000) to pick up at Mega Mall. North Door 1 is a good spot. It has a A&W inside and seems that driver should know this door. Mine called me asking whether I'm at A&W (pronounced Ah-Wei) door and if so, he'll enter immediately.
